INCIDENT REPORT

Something happened.

Nobody agrees on exactly what.

Everyone involved has a completely reasonable explanation, three witnesses suddenly remember being somewhere else, and somebody higher up would really appreciate it if we stopped using the word incident.

So naturally, there's a form for that.
